By signing my name below, I give permission for my child to participate activities offered by Life in Color Studio including art projects, baking activities, sensory play, outdoor games, nature exploration, neighbourhood walks, visits to nearby parks and playgrounds, and other supervised camp activities appropriate for children ages 5–10. I am aware that participation can include inherent risks, for example:

slips or falls
insect bites
weather
contact with plants
playground equipment
minor bumps and scrapes
